2006 Contractor Registration

Dear Contractor:

In order for you to perform work in the City of Pepper Pike during 2006, you are required to furnish the following:

1. Fill out the contractor application (PDF File).
2. Mechanical contractors furnish State Certification.
3. Certificate of insurance ($100,000/$300,000) with the City as Additional Insured. NO FAX COPIES WILL BE ACCEPTED.
4. Contractor's Bond ($25,000). Use insurance company's form. Bond must be signed before sending to our office. NO FAX COPIES WILL BE ACCEPTED.
5. If you do not have a license with the State of Ohio or have not registered with the City of Pepper Pike in the past three (3) years, you must also provide three (3) letters of reference (either from customers or other municipalities).
6. Check for $40.00.
7. Self-addressed stamped envelope.

Failure to comply with the above request will be subject to the penalties provided in Section 1440.99 City of Pepper Pike Codified Ordinances. Failure to be registered and still work in the City will result in a STOP WORK ORDER being issued by our Building Inspector, you will also be charged double when applying for your registration. If you are working without a permit a STOP WORK ORDER will be issued, and you will be charged double for your permit.
